Following the communication in the September newsletter regarding the new NHS Health Checks contract with Leicester City Council. The council has informed us that they have now completed their internal governance approval process for this service, and are looking to issue the documents onto the Proactis portal on Friday 21st November 2025.
Unfortunately, there have been some delays with this programme, but once the documents have been published the City Council representatives will begin holding the support sessions, along with support to help practices register onto the Proactis portal, where practices will be able to express interest in the NHS Health Checks service and download the documents for completion.
Attached is copy of the revised slides for your information as slides 13, 14, 15 and 16 have been updated.

The LMC hosted a webinar on Tuesday 16th September between Leicester City Council and City practices on the changes to the Leicester City Health Checks. Practices asked the City Council to provide the list of
documents practices will need to have ready prior to completing the contract application.

Health & Safety:

Finance:
- Full set of financial statements which includes your profit/loss and balance sheet statements for the last
two years if a web address is not available.
- If full financial accounts are not available, then we would require one of the following:
- a statement of annual turnover-e most recent year(s) of trading and a bank letter outlining the current
cash and credit facility position - Alternative information to evidence economic and financial standing (e.g. forecast financial statements
and a statement of funding provided by the owners and/or the bank, charity accruals accounts or an
alternative means of demonstrating financial status.

Data Protection:
- Evidence of a clear hierarchy responsible for Information/Data Protection within the organisation
- Data Protection Policy reflecting Data Protection Act 2018
- Records Management Policy and retention schedules
- Information and ICT Security Policies and process for managing information security incidents
- Staff training records and evidence of a process to refresh training regularly
- Guidelines for handling information (clear desk approach, secure offices, storage etc)
- Any compliance with ISO27001, Cyber Essential or equivalent certifications.
