Are you an experienced nurse with a passion for long-term condition management? Whether you already have experience in diabetes care or are looking to develop a specialist interest in diabetes, we’d love to hear from you.

Serving approximately 11,000 patients, we are a friendly teaching and training practice with an experienced and supportive multidisciplinary team. We have 9 GPs, up to 4 GP registrars, 5 registered nurses and a range of clinical and admin support staff. We pride ourselves on delivering high-quality, patient-centred care in a collaborative environment where staff wellbeing and professional development are valued.

The Role

This is an excellent opportunity for a motivated Practice Nurse who is passionate about long-term condition management, particularly diabetes, while providing a broad range of general practice nursing services.

Key responsibilities include:

  • Supporting the management of patients with diabetes in line with current evidence-based guidance.
  • Providing chronic disease management and health promotion.
  • Delivering childhood and adult immunisations and vaccinations.
  • Undertaking cervical cytology screening.
  • Providing wound assessment and management.
  • Supporting patients with lifestyle advice and self-management of long-term conditions.
  • Working closely with GPs, pharmacists, healthcare assistants and the wider multidisciplinary team to provide safe, effective and compassionate care.

About You

We are looking for a Registered Nurse with current NMC registration who:

  • Has experience working in primary care or managing long-term conditions.
  • Has a special interest in diabetes care, with relevant training or qualifications desirable.
  • Is competent in immunisations and vaccinations.
  • Is experienced in cervical cytology screening.
  • Has sound wound care and dressing management skills.
  • Has excellent communication and organisational skills.
  • Works well both independently and as part of a multidisciplinary team.

A non-medical prescribing qualification would be an advantage, although applications will be welcome from nurses who are interested in working towards this qualification over the next few years.
