Job summary
To be responsible for implementing processes for the effective management of patients with long-term conditions using evidence-based practice, including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the practice’s registered patient population.
The Practice Nurse will be responsible for a range of clinical areas, including infection prevention and control, chronic disease management, health promotion, and well woman and well man clinics, as well as actively supporting the practice management team in reviewing and delivering clinical policy and procedure.

Quality and continuous improvement

Induction
New starters at Severn Surgery are required to complete the practice induction programme; the practice management team will support you throughout.
Learning and development
Training and development ensure staff have the skills, knowledge and competence to perform their role. All staff must complete mandatory training as directed, and the post holder is expected to assess their own learning needs and undertake learning as appropriate.
The post holder will mentor team members, share learning and information to support good practice, and provide an educational role to patients, carers, families and colleagues.
Collaborative working
All staff should understand their role and scope within the wider team, prioritise their workload, and support effective time management. Teamwork is essential in a multidisciplinary environment, and the post holder is expected to work supportively with others, communicate clearly, delegate appropriately, and help agree plans and outcomes by which success can be measured.
Managing information
Staff should use technology and appropriate software to support the planning, delivery and monitoring of care, and to present and communicate information. Data should be recorded using accurate SNOMED codes to support reliable retrieval for monitoring and audit.
Service delivery
Staff receive detailed information on policy and procedure during induction. The post holder must adhere to the organisation’s policies and regional directives at all times.
Security
The security of the practice is the responsibility of all staff. The post holder must remain vigilant and report any suspicious activity to their line manager immediately. Door codes and passwords must never be shared, and restricted areas must remain secure.
Professional conduct
All staff are required to dress appropriately for their role.
Leave
All staff are entitled to take leave. Line managers ensure staff are able to take a minimum of four weeks’ leave each year, and staff are encouraged to use their full entitlement. Public holidays are calculated on a pro-rata basis according to hours worked.
Primary key responsibilities
The following are the core responsibilities of the Practice Nurse. There may, on occasion, be a requirement to carry out other tasks, depending on factors such as workload and staffing levels:
a.To support the Infection Prevention and Control (IPC) lead, working to the IPC Policy and supporting audits, training and evidence of compliance to the practice manager.
b.To support the IPC annual statement in conjunction with the practice manager.
c.To develop, implement and embed an effective care management programme for frail and housebound patients registered at the practice.
d.To assess the needs of patients, ensuring the provision of healthcare is appropriate and incorporates evidence-based practice.
e.To develop, implement and embed health promotion and wellbeing programmes.
f.To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
g.To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health.
h.As required, to provide routine nursing care to patients in accordance with clinical evidence, NICE guidance and the NSF.
i.As required, to diagnose and manage acute and chronic conditions.
j.To administer medication to patients in accordance with PGDs and own scope of practice.
k.To review patient medications to enhance compliance.
l.To guide patients in the use of prescribed and over-the-counter medications, including side effects and dosages.
m.To co-ordinate the provision of travel medicine services.
n.To request pathology services as required.
o.To oversee the provision of chronic disease clinics, delivering patient care as necessary and referring patients to secondary or specialist care as required.
p.To maintain accurate clinical records in accordance with extant legislation.
q.To ensure Read codes are used effectively by all members of the nursing team.
r.To ensure the appropriate maintenance and use of chronic disease registers.
s.To develop, implement, embed and participate in well woman clinics.
t.To develop, implement, embed and participate in well man clinics.
u.To ensure compliance with the practice chaperone policy.
v.To assist GPs with minor surgery when required.
w.To prioritise health issues and intervene appropriately.
x.To support the team in dealing with clinical emergencies.
y.To recognise, assess and refer patients presenting with mental health needs.
z.To implement vaccination programmes for adults and children.
aa.To support patients in the use of their prescribed or over-the-counter medicines, within own scope of practice.
bb.To liaise with external services and agencies to ensure vulnerable and other patients are supported appropriately.
cc.To delegate clinical responsibilities appropriately, ensuring safe practice and that tasks remain within the delegate’s scope of practice.
dd.To support the clinical team with all safeguarding matters, in accordance with local and national policies.
ee.To understand practice and local policies on substance abuse and addictive behaviour, referring patients appropriately.
ff.To deliver opportunistic health promotion where appropriate.
Secondary responsibilities
In addition to the primary responsibilities, the Practice Nurse may be asked to:
a.Develop and review audit protocols and processes for the practice, effectively utilising the audit cycle.
b.Develop, review and implement nursing protocols in conjunction with the partners.
c.Support all members of the nursing team, providing guidance when necessary and conducting appraisals and training needs analyses as required.
d.Participate in local initiatives to enhance service delivery and patient care.
e.Support and participate in shared learning within the practice to improve patient care.
f.Continually review clinical practice, responding to national policies and initiatives where appropriate.
g.Participate in the review of significant events and near misses, applying a structured approach such as root cause analysis (RCA).
h.Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs.
i.Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients.
j.Support the practice team in reviewing and implementing practice policies and protocols, ensuring conformance with extant legislation.
