- Key Responsibilities:
- Prescribe any medicine for any condition within competence (including some controlled medicines)
- See patients with undiagnosed, undifferentiated medical conditions and make treatment decisions, including ordering necessary investigations
- Refer patients to secondary care and other holistic therapies
- Undertake appropriate home visits and Care Home ward rounds
- Provide high-quality, patient-centred care, complete Care Plans and ReSPECT forms.
- Collaborate with the wider healthcare team
- Requirements:
- Advanced Nurse Practitioner qualification (Masters Level) - graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre’s ePortfolio (supported) Route
- Independent Prescribing qualification
- Current registration with the appropriate nursing body
- Excellent communication and interpersonal skills
