Ordering MED3 forms

We have recently had a number of queries about how to order MED3 forms.  Practices have been told that they are not available on Primary Care Support England's (PCSE) online portal within the supplies catalogue.

 

Some practices were advised that the Department of Work and Pensions (DWP) were no longer producing paper MED3 forms (Fit for Work certificates).

 

This is not the case.  Currently, practices cannot order these via the PCSE portal, although we have been told by NHS England discussions are ongoing between themselves, Capita and the DWP to make these available in the future.

 

In the mean time practices have two options:

 

1. Order paper MED3 forms directly from the DWP - click here for details of the DWP supplier and how to order directly.

 

2. Complete and print the form directly from your clinical system- many practices have already adopted this method of issuing Fit For Work certificates.  The advantage is that this is then part of the patient's medical record.
