Oakham Medical Practice is based in a thriving market town in the beautiful county of Rutland.  Market Overton is a picturesque village in the North of Rutland with a branch surgery in the village of Somerby just across the border in Leicestershire. The Practices serve 20,000 patients.

The ideal candidate will hold a recognised diabetes qualification and have experience in the initiation and management of injectable diabetes therapies and oral therapies and will be able to demonstrate knowledge and experience of managing a diabetes caseload. 

The following are the core responsibilities of the DSN.  There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • To work autonomously as a Diabetes Specialist Nurse delivering immediate or planned diabetes care to patients with complex diabetes
  • To co-ordinate the diabetes services within the Practice, networking with a multi-disciplinary team
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Initiation and monitoring and appropriate follow up of injectable therapies in line with local formulary and guidance
  • To facilitate self-management of diabetes with appropriate education
  • Support, manage, monitor and maintain the efficient and continued development of diabetes care services within the Practice
  • Support training and development of clinical practitioners to ensure the consistent delivery of high-quality care within the service
  • To facilitate the planning, development and evaluation of the diabetes service within the Practice
  • Implement and evaluate individual specialised treatment plans for diabetic patients
  • Monitoring the control of existing diabetes caseload and ensuring accurate diagnosis, care and education of all new diabetic referrals
