We are looking for a skilled and motivated Clinical Pharmacist with an Independent Prescribing qualification to join our GP surgery team.
Hockley Farm Medical Practice is based in Braunstone, Leicester and has a population of over 11,000 patients with 7 GPs, 2 Practice Nurses, 3 Pharmacists, and Health Care Assistants combined with an extensive and experienced administrative team.
We are an approved teaching practice for GP Registrars and Medical Students and a level 4 research practice.

Job Overview:
We are looking for a skilled and motivated Clinical Pharmacist to join our GP surgery team. The successful candidate will play an essential role in providing high-quality pharmaceutical care to our patients, ensuring safe and effective use of medications. As a Clinical Pharmacist, you will work closely with the clinical team, providing expert advice and support on medication management, patient consultations, and treatment optimisation. The role is not a managerial position and will be supervised by the Lead Pharmacist, who is responsible for overall team management.
Key Responsibilities:
- – Conduct medication reviews for patients, including chronic disease management and polypharmacy reviews.
- – Provide clinical input and advice to the medical team on the safe and effective use of medications.
- – Review prescribing across the practice, ensuring adherence to best practice guidelines.
- – Provide advice on therapeutic alternatives and medication adjustments in line with patient needs.
- – Manage and optimise medication systems and processes within the surgery to enhance patient care.
- – Ensure compliance with regulatory frameworks and practice policies regarding drug usage and prescribing.
- – Support patients with their medications, offering education on their conditions, medications, and lifestyle changes as necessary.
- – Prescribe medications as an Independent Prescriber within the scope of your qualifications.
- – Actively participate in clinical meetings and case discussions to support integrated patient care.
- – Liaise with community pharmacies to ensure smooth medication processes and discharge planning for patients.
- – Keep up to date with new drugs, therapies, and evidence-based guidelines relevant to primary care.
Essential Qualifications and Experience:
- – Independent Prescribing qualification (mandatory).
- – Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- – Proven experience in clinical pharmacy practice, ideally within primary care or a GP surgery setting.
- – Excellent communication and interpersonal skills to work with both clinical teams and patients.
- – Strong organisational skills and ability to prioritise workload in a busy environment.
- – Ability to work collaboratively as part of a multi-disciplinary team.
Desirable Skills:
- – Experience with electronic prescribing systems (EMIS/Web or SystmOne).
- – Knowledge of clinical guidelines and local formularies.
